The upper back jaw is one of the more challenging areas for dental implants, largely because of a structure most patients don’t think about until it comes up in a consultation: the maxillary sinus, an air-filled cavity that sits directly above the upper molars and premolars.

Why it complicates implant placement: when upper back teeth are lost, the bone in that area tends to resorb, and the sinus floor can also gradually drop lower, or “pneumatize,” into the space once occupied by tooth roots. The result is often too little bone height remaining to anchor an implant securely without risking intrusion into the sinus cavity.

What a sinus lift (sinus augmentation) does: the procedure gently lifts the sinus membrane upward and places bone grafting material into the space created beneath it, effectively building a new floor of bone with enough height and density to support an implant. Over several months, that grafted area matures into stable, functional bone.

Two approaches, depending on how much height is needed:

  • Lateral window technique, used when more significant height is needed — a small opening is made in the side of the jawbone to access the sinus space directly
  • Osteotome (crestal) technique, a less invasive approach used when only a modest lift is needed, accessed through the implant site itself, sometimes allowing implant placement in the same visit

What to expect afterward: mild swelling and some sinus-related pressure are common for the first several days, with straightforward home care and avoidance of activities like blowing your nose forcefully during initial healing. Most patients return to normal activity within a few days.

A sinus lift adds time to the overall implant timeline, typically several months for the graft to mature before implant placement, but it’s what turns “not enough bone” into a viable, long-term implant solution for the upper jaw.
